Prairie smoke has pinnately divided primary 7–17 leaflets; some smaller secondary leaflets are also inserted between some primary leaflets. They are arranged in ground rosettes and are semi-evergreen: some basal leaves remain throughout winter and often turn reddish-purple.

Flowers bloom in spring. They appear on short reddish-lilac stems 15 to 41 cm (6 to 16 inches) tall and arranged in umbels with 3 to 5 flowers. At the base of the umbels are leaf-like green to reddish purple pieces. Sometimes there are also pairs of leaf-like pieces on the stems below the umbels or on the stems of individual flowers. When flowering, the flowers are nodding downwards and most of them remain closed and bud-like except for a small opening at the bottom. The five petals are pale yellow to creamy white and filled with pink or purple, but are mostly covered by five red sepals. There are five narrow small brambles between the sepals. Hidden in the flower are the pistils and many stamens next to them .
